Scheduling Policy
Please arrive 5 minutes before your first appointment, and arrive on-time to subsequent appointments. Before your first appointment we will ask you to fill out a Confidential Health History that will be kept confidential and will not be shared with anyone without your permission. You may download this form from the above link and bring it to your first session. You may also download the Covid-19 Liability Waiver here.
As a professional courtesy, 24-hour notice is required for rescheduling or cancellations. If less than 24-hour notice is given, the client will be responsible for the session fee.
**During the COVID-19 pandemic, we are adjusting our cancellation policy to waive the cancellation fee if symptoms of any illness appear.
