Acceptable forms of payment are cash, check, credit card, or HSA card. Additionally, Gift Certificates can be purchased online with Paypal. We look forward to meeting you and answering any questions you may have!
Please arrive 15 minutes before your first appointment, and 5 minutes before subsequent appointment times. Before your first appointment we will ask you to fill out a Health History Form that will be kept confidential and will not be shared with anyone without your permission. You may download this form from the above link and bring it to your first session.
As a professional courtesy, 24-hour notice is required for rescheduling or cancellations. If less than 24-hour notice is given, the client will be responsible for the session fee.
**During the COVID-19 pandemic, we are adjusting our cancellation policy to 12-hours if symptoms of illness appear.
